Germany needs nurses, and it has built official routes to bring them in. Make it in Germany, the federal government's portal, notes around 31,000 vacant positions in care facilities (Make it in Germany). For Indian nurses, the big difference from the UK or Gulf is language: you will need German, usually to B2 level, before you can work as a fully recognised nurse.
This guide covers recognition (Anerkennung), language, visa routes and the free government-run Triple Win programme. Every fee and requirement was checked on 31 Oct 2026 against the official source linked beside it.
Quick summary
| Stage | Who runs it | Official fee or rule (checked 31 Oct 2026) |
|---|---|---|
| Recognition of your nursing qualification | The competent authority in the German state where you want to work | Varies by state, for example €170 to €845 in Brandenburg, up to €277.50 in North Rhine-Westphalia (Anerkennung in Deutschland) |
| German language | Goethe-Institut, telc and other recognised providers | B2 needed for the licence; exam fees: check the official site |
| National (D) visa | German Mission in India | €75, payable in INR (German Mission India) |
| Knowledge test or adaptation period | State authority and training providers | Costs vary; check with your authority |
Step 1: Understand recognition (Anerkennung)
Nursing is a regulated profession in Germany. To work as a registered nurse (the German title is Pflegefachfrau, Pflegefachmann or Pflegefachperson) you need your Indian qualification recognised and a licence to use the title (Make it in Germany).
Key points from the official recognition portal:
- You apply to one state, the state where you want to work. Recognition is then valid across Germany.
- You can apply from India. You do not need to live in Germany first. Do not post original documents.
- Timing: the authority confirms receipt within one month, and you receive a decision at the latest 4 months after your file is complete. It can be extended in specific cases (Anerkennung in Deutschland, Brandenburg example).
- Fees vary by state. Brandenburg lists €170 to €845. North Rhine-Westphalia lists a maximum of €277.50, plus possible extra costs for compensation measures or a knowledge test (NRW example).
Use the free Recognition Finder to get the exact authority, document list and fee range for your chosen state.
Typical documents (from the Brandenburg listing): passport, birth certificate, CV in German, your GNM or BSc diploma with transcripts showing hours and subjects, proof of work experience, your State Nursing Council registration, a German language certificate, and a police certificate and medical certificate each no more than 3 months old at submission. Translations into German are usually required. Many items can be sent later when the authority asks.
Step 2: Read your decision notice
The authority will tell you one of three things:
- Equivalent. Your training matches German training. You receive the licence once you meet the other conditions, such as language.
- Substantial differences. Common for Indian qualifications. The notice (often called a Defizitbescheid) lists the gaps.
- Equivalent but something missing, such as language proof.
If there are differences, you close them with a compensation measure: either a knowledge test (Kenntnisprüfung) or an adaptation period (Anpassungslehrgang). In North Rhine-Westphalia, for example, you can usually choose between the two.
Make it in Germany also describes an option to skip the full equivalence check and go straight to an adaptation programme. It can be quicker and cheaper, but your extra experience is not counted, so the adaptation period may be longer. Free advice on this is available from the Central Service Office for Professional Recognition (ZSBA).
Step 3: Learn German to B2
Recognition requires German at B2 on the European framework (CEFR) (Make it in Germany). Some states also ask for a specialist nursing language exam.
Commonly used exams:
- Goethe-Zertifikat B2 at Goethe-Institut / Max Mueller Bhavan centres in India (Goethe India). Fee: check the official site; the price table was not available when we checked.
- telc Deutsch B1-B2 Pflege, a nursing-specific exam with a writing task based on nursing documentation. It must be taken as a whole exam, not in parts (telc). Fee: check the official site.
Ask your state authority which certificates it accepts before you book. The German Mission in India's checklist for recognition visas asks for B2 proof not older than one year for care professions (German Mission India).
Step 4: Choose your visa route
Visa for recognition measures (Section 16d). If your notice shows gaps, you can come to Germany to complete the adaptation period or prepare for the knowledge test. Make it in Germany says this permit lasts up to 24 months, can be extended by 12 months, and allows part-time work of up to 20 hours a week, or more hours in your own field with Federal Employment Agency approval (visa for recognition). You must show you can support yourself. Make it in Germany gives 2026 figures of at least €1,200 gross (€941 net) per month for company-based measures; the German Mission in India checklist shows a different figure, so confirm with the mission when you apply.
Recognition partnership. You sign a contract with a suitable German employer and complete recognition while working, typically as a nursing assistant. This needs German at A2 or above, an agreement with your employer, and a ZAB assessment of your qualification. The permit is issued for up to 12 months and can be extended to a maximum of three years (recognition partnership).
Skilled worker visa. If your qualification is already fully recognised and you have a job offer, you apply for the work visa for qualified professionals, usually with Federal Employment Agency approval. Permits are issued for up to four years (work visa).
Visa fee: €75, payable in INR (German Mission India). Note that the EU Blue Card does not apply to nursing.
Step 5: Consider Triple Win (free)
Triple Win is run by Germany's Federal Employment Agency and GIZ. For Kerala, applications go through NORKA Roots (Triple Win Kerala). Official points:
- Open to graduates of accredited Indian nursing institutions with GNM or BSc Nursing.
- Placement, language training, technical preparation and integration help are free for applicants.
- Language training runs from A1 to B1 at GIZ-commissioned centres in Thiruvananthapuram. You need B1 for the visa, and you pass B2 in Germany.
- Your recognition is completed in Germany within about 12 months, with GIZ support.
Make it in Germany lists Triple Win for India (Kerala and Telangana). Watch norkaroots.org for selection rounds.
Typical timeline
| Stage | Official guide | Planning range |
|---|---|---|
| German A1 to B2 | No official figure | Often 9 to 15 months of serious study |
| Recognition decision | Within 4 months of a complete file | 3 to 6 months including document gathering |
| Visa | Depends on mission appointments | Weeks to months |
| Adaptation period or knowledge test | Set by your decision notice | 6 to 12 months |
Language usually takes the longest. Start German first.
Scam warning
- Germany's Fair Recruitment Healthcare seal. Make it in Germany highlights this quality seal for agencies where the employer covers recruitment costs. Ask any agency whether they hold it.
- Triple Win is free. Anyone charging you a fee to join Triple Win is not part of the official programme.
- "Guaranteed visa" offers are false. Only the German mission decides your visa.
- Indian agents must be registered. If an agent recruits you for abroad, ask for their registration certificate from the Protector General of Emigrants and verify it on eMigrate.
- Do not buy language certificates. Authorities check them, and fake certificates can end your application.

FAQ
Can I work in Germany with B1 German? You can enter on some routes with lower levels (A2 for a recognition partnership, B1 for Triple Win). Full recognition as a registered nurse needs B2.
Is GNM accepted in Germany? The state authority assesses each qualification. GNM holders often receive a notice with gaps to close through a knowledge test or adaptation period. Triple Win Kerala accepts GNM and BSc graduates.
How much does recognition cost? It depends on the state. Brandenburg lists €170 to €845 and North Rhine-Westphalia up to €277.50, plus any compensation measure. Use the Recognition Finder for your state.
